How to correct or access your information
You may request access to all your personally identifiable information maintained in our data base from your patient portal. If you are not signed up for the patient portal and wish to be please call the office for instructions.

We reserve the right to disclose information when legally compelled to do so or in good faith, it is believed that the law requires it or for the protection of our legal rights. We also reserve the right to disclose account information when it is believed that disclosing this information is necessary to identify, contact or bring legal action against anyone who is possibly violating our Terms of Service or to protect the safety of the general public, and those who visit our site.
