Frequently Asked Questions

How do I make an appointment?

Call the office at 931-774-4001, fill out the contact us form at the top of the page, or have your Primary Care Physician or any of your Physicians fill out the Refer a Patient form at the top of the page. Once the information is received our staff will contact you.

What should I expect at the first appointment?

The first appointment is usually the longest appointment allow for extra time for this visit.  Bring your insurance card and valid ID. You will provided patient rights and responsibilities, HIPPA forms, new patient intake and screening forms, and consent to treatment. 

Who can come with me to my appointments?

Any person who you give consent can come with you to your appointments. This is usually a family member, friend, or support person. All Minor children under the age of 16 must be accompanied by an parent or legal guardian and must stay at a minimum in the waiting room for the duration of the visit

Do I need a referral?

You are only required to have a referral if your insurance requires it. Please call your insurance company to ask if it is needed for them to cover behavioral health services

Does insurance cover behavioral health?

All insurance companies have different benefits for behavioral health services. Please call your insurance company and ask them if these services are covered under your plan

What payment methods are accepted?

We accept all major credit and debit cards, online payments, and payments submitted by mailed in statements by personal check or cashier's check. We do not accept American Express
